Choir Registration / Renewal

  1. Log on to
  2. Use the choir TAB
  3. For new registration> click Choir registration
  4. For Renewal go to Step 7
  5. Fill the Mandatory Fields
  6. After Registration
  7. Use the username and password to login. (Used in 2015-18 registration)
  8. Add new members/ delete members (mention old PCM No for refference if the member was previously in another group)
  9. Edit or modify the details if required
  10. Proceed to payment, Pay through IMPS, NEFT of bank remittance.(Payment gateway integration is under processing)
  11. Update the payment slip and Submit.
  12. Download and Print the form duly signed by the vicar and choir leader. Certificate from the Vicar is Required only for newly added choirs and members.
  13. Send the hard copy by registered post / courier along with PP Size Photo and Other documents to
    Sruti School of Liturgical Music,
    Orthodox Seminary,
    Kottayam – 686001
  14. Write Private /Parish Choir Registration on the top of the envelope
  15. You can check the status of the choir on approved choir list on website. If it is not there call sruti office for the details/ clarification
  16. ID cards and Certificates will be issue only at orientation class arranged by Sruti (zone wise or diocese wise April-May)

Documents Required for Private choir Registration/ Renewal


  • Application Form
  • Passport Size Photo /EMAIL SOFT COPY  (FOR NEW REGISTRATION and new members ONLY)
  • Copy of Bank Remittance Slip
  • Original badge (for the renewal)
